En l'absence de FKBP12, le Temsirolimus inhibe puissamment l'activité kinase de mTOR avec une IC50 de 1,76 μM, similaire à celle de la rapamycine avec une IC50 de 1,74 μM. Le traitement avec ce composé à des concentrations nanomolaires (10 nM à <5 μM) présente une activité antiproliférative modeste et sélective via un mécanisme dépendant de la FKBP12, mais peut complètement inhiber la prolifération d'un large panel de cellules tumorales à des concentrations micromolaires faibles (5-15 μM), impliquant une suppression indépendante de la FKBP12 de la signalisation mTOR. Ce traitement chimique à des concentrations micromolaires mais non nanomolaires (20 μM) provoque une diminution marquée de la synthèse globale des protéines et le désassemblage des polyribosomes, accompagné d'une augmentation rapide de la phosphorylation du facteur d'élongation de la traduction eEF2 et du facteur d'initiation de la traduction eIF2A. Il inhibe la phosphorylation de la protéine ribosomale S6, plus puissamment dans les cellules DU145 PTEN-positives que dans les cellules PC-3 PTEN-négatives, et inhibe la croissance cellulaire et la survie clonogénique des deux types de cellules de manière concentration-dépendante. Ce composé (100 ng/mL) inhibe puissamment la prolifération et induit l'apoptosis dans les cellules primaires de leucémie lymphoblastique humaine (ALL).

Les constructions d'ADN de mTOR humain de type sauvage marqué par Flag (Flag-mTOR) sont transfectées transitoirement dans des cellules HEK293. L'extraction des protéines et la purification de Flag-mTOR sont effectuées 48 heures plus tard. Des essais kinase in vitro de Flag-mTOR purifié en présence de diverses concentrations de Temsirolimus sans FKBP12 sont réalisés dans une plaque à 96 puits et détectés par dosage immunofluorométrique à amplification par dissociation des lanthanides (DELFIA) en utilisant His6-S6K1 comme substrat. Les enzymes sont d'abord diluées dans le tampon d'essai kinase (10 mM Hepes (pH 7,4), 50 mM NaCl, 50 mM β-glycérophosphate, 10 mM MnCl2, 0,5 mM DTT, 0,25 μM microcystine LR, et 100 μg/mL BSA). Dans chaque puits, 12 μL de l'enzyme diluée sont mélangés brièvement avec 0,5 μL de ce composé. La réaction kinase est initiée en ajoutant 12,5 μL de tampon d'essai kinase contenant de l'ATP et de His6-S6K pour donner un volume de réaction final de 25 μL contenant 800 ng/mL de FLAG-mTOR, 100 μM d'ATP et 1,25 μM de His6-S6K. La plaque de réaction est incubée pendant 2 heures (linéaire de 1 à 6 heures) à température ambiante avec une agitation douce, puis la réaction est arrêtée en ajoutant 25 μL de tampon d'arrêt (20 mM Hepes (pH 7,4), 20 mM EDTA et 20 mM EGTA). La détection DELFIA de His6-S6K phosphorylé (Thr-389) est effectuée à température ambiante en utilisant un anticorps monoclonal anti-P(T389)-p70S6K marqué à l'Europium-N1-ITC (Eu) (10,4 Eu par anticorps). 45 μL du mélange de réaction kinase stoppé sont transférés dans une plaque MaxiSorp contenant 55 μL de PBS. Le His6-S6K est laissé se fixer pendant 2 heures, après quoi les puits sont aspirés et lavés une fois avec du PBS. 100 μL de tampon DELFIA avec 40 ng/mL d'anticorps Eu-P(T389)-S6K sont ajoutés. La liaison de l'anticorps se poursuit pendant 1 heure avec une agitation douce. Les puits sont ensuite aspirés et lavés quatre fois avec du PBS contenant 0,05 % de Tween 20 (PBST). 100 μL de solution d'amélioration DELFIA sont ajoutés à chaque puits et les plaques sont lues dans un lecteur de plaques PerkinElmer Victor.

Dans les modèles de xénogreffe NOD/SCID avec ALL humaine, le traitement au Temsirolimus à 10 mg/kg/jour entraîne une diminution des blastes sanguins périphériques et de la splénomégalie . L'administration de ce composé (20 mg/kg i.p. 5 jours/semaine) retarde significativement la croissance des xénogreffes DAOY de 160 % après 1 semaine et de 240 % après 2 semaines, par rapport aux contrôles. Une dose unique élevée de ce produit chimique (100 mg/kg i.p) induit une régression de 37 % du volume tumoral en 1 semaine. Le traitement par ce composé pendant 2 semaines retarde également la croissance des xénogreffes U251 résistantes à la rapamycine de 148 %. L'inhibition de mTOR par cet agent améliore les performances dans quatre tâches comportementales différentes et diminue la formation d'agrégats dans un modèle murin de la maladie de Huntington. L'administration de ce composé induit des réponses antitumorales significatives et dose-dépendantes contre la croissance sous-cutanée des xénogreffes 8226, OPM-2 et U266 avec des ED50 de 20 mg/kg et 2 mg/kg pour 8226 et OPM-2, respectivement, qui sont associées à une prolifération et une angiogenèse inhibées, à l'induction de l'apoptosis et à une réduction de la taille des cellules tumorales.
